Dear parent or guardian
An NHS team is coming to school name on next session dates to give the vaccine.
They would like your consent to vaccinate child’s name.
You can give this by filling in the online form.
You can use the online form to:
- find out more about the vaccine
- give consent for your child to be vaccinated
- say you do not want your child to be vaccinated
Filling in the form will be quick and easy. It’s important to respond, even if you do not want your child to be vaccinated.

It’s important to confirm whether you do or do not want your child to have the vaccination. It will take less than 5 minutes to respond using the link below.

You need to respond by consent deadline.
If you cannot use the online form, you can respond over the phone using the contact details below. Replies to this email cannot be accepted as consent.
Keeping your information safe
If you consent, child’s name vaccination will be recorded in line with NHS data security standards so their vaccination record is up to date, but no contact details will be shown.
