Our NHS team is coming to name of school on next session dates to give the vaccination.
We have no contact details for one or more children at your school who are eligible for the vaccination. This is often because their parents have said they do not wish the child’s details to be shared.
What we need from you
We’d like you to contact the parent(s) or guardian(s) of name(s) of child(ren) to ask them to complete our online consent form.
You can copy and edit this example email template. Under the How to respond heading you need to add:
- the link to the consent form: link to consent form for relevant vaccination
- the consent deadline: consent deadline
