Sharing consent forms with schools who have not provided contact details

If schools have not provided parents’ contact details, they’ll need to send out consent requests that you’ve provided them with.

You can use the following email templates, adapting as necessary before sending them to the school:

• Initial consent request for flu
• Initial consent request for HPV
• Initial consent request for MenACWY and TD/IPV

You’ll also need to tell the schools when they should send the requests.

To get the URL for the consent form:

1. Go to Sessions.
2. Filter for Scheduled sessions.
3. Select the relevant school.
4. Scroll down to the links for the consent forms for the different vaccination programmes.
5. Select the relevant online consent form link to get the URL you need to use in the email template.

Adding MMR(V)

If you receive consent for a vaccination and your team is also running MMR or MMRV in the same session as that programme, you’ll then have the parent’s contact details and can send them a consent form for MMR or MMRV. This is an operational decision for your team.
